Aim: Immunofixation electrophoresis (IFE) is a gold standard biochemical technique used in the diagnosis, treatment and follow-up of monoclonal gammopathies. In paraproteinemias, excessive production of immunoglobulins (Ig) known as paraprotein or monoclonal protein (M-protein) occurs. Mprotein can be seen as a sharp band in serum or urine electrophoresis. Paraprotein types produced in monoclonal gammopathies can be defined by IFE method. In this study, we aimed to retrospectively evaluate the types of paraproteinemia detected by the IFE method in the laboratory and to compare the findings with the literature.

Material and Methods: In this study, a total of 403 serum IFE analysis reports performed in the laboratory were evaluated retrospectively. The laboratory information system was scanned and data of the patients were obtained. Serum and urine samples of patients were performed by using Interlab G26 analyzer (Interlab Srl, Rome, Italy).

Results: In the study, paraprotein band was detected in 96 patients' serum (23%). IgG kappa (33%) was detected in 32 patients and IgG lambda (20%) in 19 patients. The presence of paraprotein band was found to be 23% in patients who underwent urine IFE.

Conclusion: In our study, we found the most IgG kappa and secondly IgG lambda paraproteinemia types similar to the literature. Our study presented an important projection on the frequency of monoclonal gammopathy in the laboratory and aimed to contribute to the predictability of the paraprotein type. Performing serum and urine IFE analysis at the same time will contribute in the diagnosis of patients with paraprotein band detected in serum protein electrophoresis (SPE) or who are thought to have paraproteinemia clinically.
