Serum protein electrophoresis (SPEP) is the first line test in the diagnosis of a wide range of diseases including multiple myeloma, that are classified as monoclonal gammopathies. The test is one of the best fitting to the clinical title of the clinical biochemists as it urges the complete evaluation of clinical, radiologic and laboratory tests of the patient and a close relationship with the clinicians who request SPEP. Standardization and harmonization in reporting SPEP is a current problem in both Turkey and the World. This paper followed the Diagnosis and Management of Multiple Myeloma Guide by Turkish Hematology Association in order to compose a minimal requirements background in reporting the test. Personal experiences were shared besides the literature in order to offer a flowchart for the laboratory specialist in evaluating SPEP.
